Pfizer (NYSE: PFE) reported Q2 2026 adjusted EPS of $0.77, beating the $0.69 consensus estimate by 11.4%, while revenue of $15.0 billion rose 2.6% year over year. A $4.3 billion non-cash intangible asset impairment charge drove GAAP net income to a loss of $248 million, compared to a $2.91 billion profit in Q2 2025, pushing GAAP EPS to $(0.04) from $0.51 a year ago.

What Drove the Results

Adjusted EPS of $0.77 beat the $0.69 consensus estimate by approximately 11.4%. Revenue of $15.03 billion grew 2.6% year over year, and excluding Comirnaty and Paxlovid, revenues grew 5% operationally, as non-COVID franchises absorbed a COVID-19 revenue decline.

The divergence between GAAP and adjusted results is the most consequential line in the quarter. The $4.3 billion non-cash intangible asset impairment recorded in Q2 2026 flipped GAAP net income from a $2.91 billion profit to a $248 million loss, with no comparable prior-year charge. Adjusted income, which strips out non-cash items, came in at $4.44 billion versus $4.43 billion a year ago, essentially flat.

Key product-level growth drivers included Eliquis, up 19% operationally globally on higher net pricing and demand; Padcev, up 23% operationally on expanding market share in urothelial cancer; Lorbrena, up 37% operationally in first-line ALK-positive non-small cell lung cancer; and the Vyndaqel family, up 8% operationally on continued patient diagnosis uptake internationally. COVID-19 product revenues declined, with full-year 2026 COVID revenue expectations revised down to approximately $4 billion from approximately $5 billion previously.

Guidance and Capital Allocation

Pfizer raised its full-year 2026 revenue guidance midpoint by $500 million to a range of $60.5 billion to $62.5 billion, up from the prior range of $59.5 billion to $62.5 billion. The revision reflects approximately $1.5 billion in better-than-expected non-COVID product performance, partially offset by the $1 billion reduction in COVID-19 revenue expectations. Full-year 2026 adjusted diluted EPS guidance was reaffirmed at $2.80 to $3.00, absorbing an approximately $0.10 unfavorable impact from a $650 million acquired in-process R&D charge tied to the completed Innovent Biologics licensing transaction, to be recorded in Q3 2026.

The company also announced an additional $2.5 billion in anticipated productivity enhancement savings, expected to be realized from 2027 through 2029. During the first six months of 2026, Pfizer paid $4.9 billion in cash dividends ($0.86 per share) and invested $5.3 billion in internal R&D. No share repurchases have been completed in 2026, and current guidance does not anticipate any.

Analyst consensus heading into the print leaned toward a Hold-heavy distribution, consistent with the stock's multi-year underperformance relative to broader health care indices. The adjusted EPS beat of 11.4% and the guidance raise at the midpoint provide a cleaner fundamental picture than the GAAP loss suggests.

Investor Takeaway

The $4.3 billion impairment charge creates a headline GAAP loss that obscures what was operationally a solid quarter: 18% operational revenue growth from launched and acquired products and an adjusted EPS beat of 11.4% over consensus. The more forward-looking signal is the guidance raise, which explicitly credits non-COVID franchises for $1.5 billion in outperformance, suggesting the portfolio transition away from COVID dependency is advancing on a measurable basis. The Q3 Innovent Biologics charge of $650 million is already absorbed in full-year EPS guidance, limiting incremental downside to the adjusted earnings outlook from that transaction.
